Osteopathy After Knee Reconstruction (ACL): A Rehab Guide

How osteopathy treatment complements physiotherapy after an ACL reconstruction — protecting the graft while restoring whole-body function.

A long road, done properly

ACL reconstruction is a 9–12 month rehabilitation, not a 9–12 week one. Return-to-sport too early is the leading cause of re-rupture, and swelling, quadriceps shutdown and altered gait can linger for months if not managed. Osteopathy treatment sits alongside your surgeon and physiotherapist's protocol — we don't replace either, but we address the parts of recovery that pure exercise-based rehab can miss.

Where osteopathy adds value

Once the surgeon has cleared hands-on care (usually 2–3 weeks post-op), our Mitcham osteopaths help in four ways. First, reducing residual swelling with gentle lymphatic drainage and soft-tissue work around the calf, thigh and popliteal fossa. Second, restoring the mobility of the surrounding joints — the ankle, hip and lumbar spine all stiffen from limping and crutches, and freeing them up removes stress from the healing knee. Third, waking up a dormant quadriceps by treating the surrounding tissues and pairing hands-on work with targeted activation drills. Fourth, scar management once the wound is fully closed.

Rehab milestones

Full extension by week two, single-leg balance by week six, comfortable single-leg squat by three months, running program from around four to six months (surgeon dependent), change of direction and pivoting from six to nine months, and return-to-sport testing at nine to twelve months. We track your progress and only progress load when the tissues, strength and control support it.

The long view

A well-rehabbed knee reconstruction can be stronger and more resilient than the pre-injury version. Rushing steals that outcome.
